I'm Doing Everything Right... So Why Won’t This Belly Fat Budge?

I can’t tell you how many times I’ve heard this , usually after a woman sighs deeply, her arms folded over her abdomen, frustration all over her face.

“Dr. Peoples, I’ve changed nothing. I eat right. I work out. I don’t even drink like I used to. But this belly fat just… won’t… move.”

She’s not lazy. She’s not uninformed. She’s you. She’s me.

Let me tell you about someone I saw recently , we’ll call her Renee. She’s 46, works full-time, meal preps on Sundays, does Pilates twice a week, and hits the treadmill on weekends. She doesn’t snack at night. She gave up sugar months ago. And still, she sat in my office almost in tears, wondering why she didn’t feel like herself anymore.

The clothes fit differently. The reflection in the mirror felt unfamiliar. And worst of all? The advice out there wasn’t helping. Just more noise about “calories in, calories out,” intermittent fasting, or adding more HIIT.

But here’s what I told Renee , and what I want to tell you:

This isn’t just about your habits. It’s about your hormones.

Your Body Is Not Betraying You , It’s Adapting

As we enter perimenopause, our hormones , particularly estrogen, progesterone, and testosterone , begin to shift. Often in quiet, sneaky ways at first. But the body always tells the truth.

Estrogen, which used to help store fat around the hips and thighs, begins to decline. In response, fat starts redistributing to the belly. Why? Because abdominal fat can actually make a small amount of estrogen , and your body is trying to help you.

Testosterone and progesterone also drop, which can lead to muscle loss, insulin resistance, and increased fat storage , especially around the midsection. These are changes I can literally see on labs and body composition scans, even in women who are “doing everything right.”

And the wildest part? You can be gaining fat and losing muscle at the same time. So the scale may barely move, but your clothes feel tighter, your energy dips, and you feel… different.

So What Do We Do?

The truth is, belly fat in this stage of life is complex. It’s not just a diet problem. It’s a hormonal, metabolic, and lifestyle issue.

Here’s how I work with women like Renee:

1. We get clear on what’s really going on.

We run labs to check hormone levels , but I also listen to your symptoms. I want to know how you’re sleeping, how your mood is, how your cycle has changed. The body speaks in patterns, not just numbers.

We also use InBody composition scans to look beyond weight and see where your fat is being stored, how much muscle you’ve got, and what your basal metabolic rate is. This is how we track real change.

2. We personalize the lifestyle , and add structure.

You don’t need another generic meal plan or a challenge that only lasts 10 days. You need sustainable shifts, built around your life and your hormones.

Most women I work with need more protein , targeted resistance training , blood sugar balance , and realistic goals that we hold them accountable to. Because the truth is: what worked in your 30s just won’t work the same in your 40s. Your strategy has to evolve with your biology.

3. We support your body from the inside out.

Many women benefit from bioidentical hormone replacement therapy (BHRT) . Not because they’re broken , but because their bodies are changing. BHRT helps restore estrogen, progesterone, and testosterone to optimal levels, which research has shown can:

  • Reduce visceral (belly) fat and improve insulin sensitivity ( Menopause , 2010)
  • Increase lean muscle mass and energy ( JCEM , 2006)
  • We may also use nutraceuticals like B12 and NAD+ injections , which support mitochondrial health, metabolism, and energy production , especially helpful when your cells are running low after years of stress and adaptation.

This Isn’t About “Getting Skinny.” It’s About Getting Free.

When I work with women like Renee, the goal isn’t just a flatter stomach. It’s confidence. It’s peace. It’s feeling strong and clear and you again.

Your belly isn’t betraying you. It’s communicating with you.

The question is , are you ready to listen, and get the support you need to respond with care, science, and strategy?

This is the kind of medicine we practice at Peoples Wellness and Longevity .

    Not quick fixes.

    Not shame.

Just truth, tools, and a team to walk beside you.

And if that’s the kind of care you’ve been searching for , I’d love to meet you there.

    With love and truth,

Dr. Nicole Peoples Founder, Peoples Wellness and Longevity
